Application Process
Steps
  1. Complete the application and return by mail or fax it to 781-455-0892.
  2. Complete the CORI Form and return it to the Health Department in person at your earliest convenience. We will need to see your drivers’ license at this time to verify your identity in person.
  3. Once we receive your application and CORI form, you will be appointed to the Medical Reserve Corps (MRC) by the Board of Health, we will notify you of your appointment by mail or email.
  4. After you are appointed to the MRC, you must go to the Town Hall and be sworn in by the Town Clerk's Office. (Hours: Monday to Friday 8:30 a.m. to 5 p.m. and the second and fourth Tuesdays of the month 7:30 p.m. - 9 p.m.).
  5. You will receive a certificate of appointment upon being sworn in.
  6. We will set up a time to have a picture taken for a MRC Badge.
  7. The above process will officially provide town coverage for your actions in an emergency when you are under the direction of the Needham Health Department.
