Changes in Coverage
In general, if you want to enroll in or make any changes to your health, dental, life, you may do so only during the annual open enrollment period. The effective date of the change will be July 1 of each year. It is always a good idea to review your life insurance beneficiaries periodically and you may make changes to your beneficiaries at any time.

Qualifying Events
The only other time that you may make a change is if you have a qualifying event. You must complete a change in election form and attach all necessary documentation. This must be done 30 days from the date that the event occurred. Qualifying events are the following:

Qualifying Event
Legal Marital Status
Events that change my legal marital status including marriage, death of a spouse, divorce, legal separation, or annulment 
Number of Dependents
Events that change the number of my dependents including birth, adoption, placement for adoption, or death of a dependent 
Employment Status
A termination or commencement of my employment or the employment of my spouse or dependent or a significant change in medical benefits or premiums attributable to my own or my spouse’s employment 
Work Schedule
A reduction or increase in my hours of employment or those of my spouse or dependent, including a switch between part time and full time, a strike or lockout, or the beginning of or return from an unpaid leave of absence 
Dependent Satisfies or Ceases to Satisfy the Requirements for Unmarried Dependents
Dependent satisfies or ceases to satisfy the requirements for unmarried dependents 
Residence or Work Site
A change in the place of residence or work of the employee, spouse, or dependent 
Loss of COBRA coverage 
Other events authorized by law and approved by the plan administrator
